Making Real-Time Referrals
We recognize that sometimes providers want to make referrals for their patients in-session. To support this need, we've centralized available options for Higher Levels of Care (HLOC) and Adjunct Services on our Partner Network Community Page.
This page is designed to help you quickly identify suitable programs for your patients based on clinical needs, insurance coverage, and location in real-time. It serves as your single source for the most up-to-date program details and referral instructions.

These resources are a per-provider referral basis and are not coordinated by Tava directly.
Important: Never send this resource link directly to patients. Instead, we recommend contacting the partner yourself to start the process of referring your patient. However, if you feel your client is capable and willing to do the process themselves, be sure to share relevant information by providing a screenshot or sending the necessary facility contact details found on the page.
Contacting the referral partner yourself will ensure that after client completes care they can return to you as their provider to ensure continuity of care.
Tava Partner Network
We've established relationships with several organizations to make it easy to send patients to programs you can trust. The most current details, downloadable one-pagers, and referral forms for all partners are available on the Partner Network Community Page.
Charlie Health
Tava maintains an informal partnership with Charlie Health to expand access to a higher level of care for your clients. If you have clients who need a higher level of care, you can connect with Charlie Health’s virtual Intensive Outpatient Program (IOP).
Overview of Services:
- Virtual IOP (Intensive Outpatient Program): For clients ages 8-64 who need more than weekly therapy or are stepping down from inpatient/PHP/ER care.
- Treatment: Offers evidence-based group, individual, family, and psychiatric support.
- Paperwork: Charlie Health Providers are able to complete any necessary paperwork for their clients (i.e. FMLA or Short Term Disability).

How to Refer
- Gather Client Information: Name, age, best contact details, primary diagnosis/level of care (e.g., stepping down from inpatient), and insurance coverage (commercial plans, Medicaid in certain states, or self-pay).
- Submit Referral: Use the online form or phone number listed above.
- Follow Up: Charlie Health’s Admissions Team handles intake, insurance verification, and scheduling. They provide periodic treatment updates and a final clinical summary at discharge, with client consent.
Acadia Healthcare
Acadia Healthcare offers a comprehensive network of behavioral health treatment facilities, including inpatient, residential, and outpatient programs across the U.S. They have 262 locations in 39 states.
Key Resource: Acadia utilizes Treatment Placement Specialists (TPS) who assist providers in navigating their network to find the most appropriate level of care for a client. This service is often complimentary to the referring professional.

How To Refer
- Identify a patient or client in need of an alternate level of care.
- Contact your TPS (April Washburn) directly via phone, email, or fax.
- Provide items needed for referral, such as client name and demographics, clinical summary, and timeline.
- Receive personalized treatment recommendations designed to align with your clients’ clinical needs, health insurance coverage, and transportation and logistical factors, ensuring a seamless and effective experience.
General Acadia Referral Process: The streamlined referral process typically begins with a call with a TPS like April. April will help identify the Acadia facility most appropriate for the patient’s care and assist in the process of referring the patient to that facility. April is a valuable resource with strong connections to higher levels of care and readily assists in referring patients when Acadia’s programs aren’t a fit.
