How to Complete Your CAQH Profile

To finish your Tava Health account setup, be sure to complete these steps on your CAQH: two professional references, your education history, and your birth city, state, and country. This article walks you through adding each one, then re-attesting and returning to Tava.

Before you start

Have this information ready and sign in to CAQH ProView:

  • Two professional references: provider type, first name, last name, email address, and phone number for each
  • Education: school, years attended, and degree for each undergraduate and graduate degree
  • Birth details: birth city, birth state, and birth country

Step 1: Add your professional references

CAQH requires two professional references. Go to Professional References in your CAQH profile and complete these mandatory fields for each reference:

  • Provider Type
  • First Name and Last Name
  • Email Address
  • Phone Number

Step 2: Add your undergraduate and graduate degrees

Go to Education & Professional Training, then select Add next to "Enter an education record." Enter the school, years attended, and degree for each undergraduate and graduate degree you hold.

Step 3: Add your birth city, state, and country

Go to Personal Information and scroll to the Demographics section. Fill in Birth City, Birth State, and Birth Country.

Step 4: Save and re-attest

After you add the references, education, and birth details, save your changes and re-attest your CAQH profile.

Step 5: Finish your Tava setup

Click here to finish your Tava setup.

If you aren't prompted with next steps when you sign in, select Go on the Account Setup card in the upper right of your Schedule.

 

Why is this information required by Tava, if it isn’t mandatory in CAQH?

Tava requires some less common pieces of information because of our delegated credentialing agreements with payers, which means you’ll wait less time for credentialing and enrollment.
