Easily send assessments to your clients, and rely on Tava to send regular assessment requests on your behalf.
In May 2024, Tava replaced the Core-10 assessment in client intake with the new Tava Mental Health Assessment, which is a combination of:
- GAD-7 (Anxiety)
- PHQ-9 (Depression)
- CSSRS (Suicidality Risk), if applicable based on their answer to the last question of PHQ-9
All clients will take the assessment when they first set up their Tava account.

- Both you and your client can view assessment scores and responses anytime.
- Clients can retake the assessment whenever needed.

During a video session, a quick overview of the client’s recent assessments is available in the client info drawer.
Ongoing Assessments & Automated prompts
It’s recommended that clients complete the Tava Mental Health Assessment about once a month while actively in therapy.
Clients automatically receive an assessment reminder every 4 weeks, as long as:
- They have logged in within the past 4 weeks.
- They haven’t already taken an assessment during that time.
You can customize the text of the automated message in your preferences.
Need to turn off automated reminders for a client?
You can disable prompts for individual clients in their Assessments tab using the toggle at the bottom of their profile.
Manually Sending Assessments
You can also manually send your clients the Mental Health Assessment or Suicidality Risk Assessment anytime in a message.
Reviewing Client Progress
When a client completes an assessment, you’ll receive a notification with a link to their Progress Dashboard.Their dashboard includes all completed assessments, including any Core-10 assessments taken before May 2024.
Clicking on an Assessment Response will show the client’s detailed answers. CSSRS (Suicide Risk) assessments will only appear in the list format, not in the progress chart.
