If you plan to stop providing therapy on Tava Health, follow the steps below to help protect continuity of care, complete documentation, and receive any remaining payments and tax documents. This article covers provider-requested account deactivation. Tava-initiated account changes may follow a different process.
Before requesting deactivation
Important: If you choose to deactivate your account within your first year on the platform, you are not eligible to rejoin for one full calendar year after deactivation.
Consider whether you need a temporary pause instead of full deactivation. You can keep your account active, and simply turn off Tava Referrals, clear your availability, and add away time as needed. You can adjust these settings later on when you are ready to resume seeing clients.
If you want to fully leave Tava Health, contact therapist-support@tavahealth.com or request human assistance through the support chat bubble. There is no self-serve deactivation option. Please be sure to include:
The reason you are leaving
Your anticipated last day on the platform
The email address associated with your provider account
The Tava Health clinical team will review your account and confirm the effective deactivation date. Do not assume your access ends when you submit the request; continue using the platform until the confirmed effective date unless Tava Health tells you otherwise.
Account deactivation checklist
- Complete documentation: Finalize all session notes and other required documentation. Notes should be completed within 14 days of the scheduled session. Notes completed 30 days or more after the scheduled session are identified as undocumented and are no longer eligible for compensation. See Payouts for current documentation and payment deadlines.
- Plan for active clients: Notify active clients about your transition and, when appropriate, schedule a closing session. Work with Tava Health to document each client’s care status and arrange a transition to another Tava Health provider when ongoing care is needed. You cannot direct Tava-referred clients to your own practice.
- Stop accepting new referrals: Turn off new Tava referrals in your provider preferences. Clear or adjust your availability and add away time to your calendar as needed so that new appointments are not scheduled during your transition.

Confirm tax and payment information: Before your access ends, verify that your mailing address, tax identification information, and W-9 information are current. Select the gear icon on the payouts tab to check your tax ID and W-9 information.
Important: Deactivated providers cannot update address, tax ID or W-9 information in the platform. If your mailing address changes after deactivation, contact therapist-support@tavahealth.com as soon as possible.
What happens after deactivation
On the confirmed effective date:
You will no longer be able to log in to or use the Tava Health platform.
Your Tava Health account will be deactivated, not deleted. Clinical records and other health information must be retained for legally required periods and cannot be permanently deleted on request.
Your public Tava Health provider profile and referral access will no longer be active, but may continue to show up in web search results for a few days until search engines have reindexed the page.
You will be unenrolled from all payer contracts under Tava Health.
Final payments
Your final payments follow Tava Health’s standard payment schedule:
Sessions with notes completed from the 1st through the 15th are paid on the 25th.
Sessions with notes completed from the 16th through the end of the month are paid on the 10th of the following month.
Notes must be completed within 14 days of the scheduled session. Notes completed 30 days or more after the session are not eligible for compensation.

Form 1099
Form 1099 information is provided at the beginning of the following year. For deactivated provider profiles, Tava Health automatically mails a paper copy to the mailing address on file. Keep your address current before deactivation, or contact therapist-support@tavahealth.com if it changes as soon as possible.

Rejoining Tava Health
If you deactivate your Tava Health account within your first year on the platform, you are not eligible to rejoin for one full calendar year after deactivation.
If you are eligible to return, contact therapist-support@tavahealth.com. Tava Health will confirm whether your account can be reactivated and required next steps. Please note that you will need to complete all onboarding steps again, and re-enrollment with insurance payers can take up to 4 months to complete.
